Please note in order to email/fax, you need to use the proxy sent to you by mail. The cut-off time is Friday, August 12, 2016 at 12:00pm.
1) CALL 1-888-518-6805 (RECOMMENDED)
2) If voting by INTERNET (RECOMMENDED):
- Go to www.investorvote.com
- Enter your NEW 15-digit control number found on the bottom left-hand corner of the new form of proxy you received in the mail
- Under heading ‘Appointment of Proxyholder’, check the ‘Name of other appointee’ box and enter EXACTLY AS THEY APPEAR any of the following doctors to represent your proxy at the meeting

How do I find my control number?
Your control number was provided to you in the email you received. Be sure you are using the second control number you received from the OMA. If you did not receive the email please call Computershare to retrieve your control number, follow these steps:
- Call Computershare at 1-800-564-6253
- When asked by the automated system, state “I am not a shareholder”.
- The call will be transferred to the call center, and the representative who answers the call needs to be told
- the caller is attempting to retrieve their control number for voting the OMA meeting.
- They will be asked questions to confirm their identity.
- They will then be given a 15-digit control number to enter at www.investorvote.com to vote.
